Nursery - Disability Access Fund (DAF) Registration form

3&4-year-old children who are in receipt of child Disability Living Allowance and are receiving the free entitlement are eligible for the Disability Access Fund (DAF).

To qualify for this additional funding your child must be in receipt of 15 hours free childcare and be in receipt of Disability Living Allowance for Children.

If you believe that your child may qualify for the DAF please complete this form and bring a copy of your DLA award letter to the school office. This document is required to enable the setting to claim this additional funding for your child.

Is your child in receipt of DLA?

DAF is paid to the provider to use for your child’s benefit as a fixed annual payment per eligible child. If you are using more than one provider you must select one provider only to receive this money, it cannot be split.
