Please read the information below and complete the medical and dietary form by Friday 18th September. This form is to be completed for all children that require a prescribed medication, inlcuding travel sickness tablets.
Please note that if any medications are required by your child you will also need to complete a ‘Parental Agreement for Setting to Administer Medication’ form
